Health Insurance:

Blue Cross Blue Shield has the following plans available to those under the age of 65. We also have Medicare Part D and Supplements available for those over 65.:

1. Insta-Care - This is short-term coverage that can be used to fill in the gaps between other coverage. (Moving from one plan to another, transition between jobs, college kids looking for their first job, etc.)

2. Go-Blue - This is for people who want a low-cost plan, including generic drug coverage and protection against major illness or injury.

3. Simply Blue - This is a policy that offers first dollar coverage for medical expenses and covers generic drugs, both with no deductible. It also offers some benefit for annual checkups and eye exams. Once the first dollar benefits are used a deductible will apply. Premiums are based on age, smoker/non-smoker, and your choice of deductible. The premiums for this policy are very competitive and everyone likes the fact that they are getting some first dollar coverage for their premium dollars. The Accord Network* is the only network available with this policy.

4. Personal Blue - This is a major medical policy with a deductible and co-insurance. Premiums are based on age, smoker/non-smoker, and your choice of deductible and network* (Aware/Accord). This policy offers limited maternity coverage. This has three options to choose from.

a. 80 with copay - this is for those coming from a group plan looking for lower-deductible plans and office visit copays.
b. 80 - this is for individuals and families looking for a traditional plan with great up-front benefits.
c. 100 - this plan if for people seeking a lower premium plan and protection from catastrophic costs.

5. Options Blue - This is a policy that offers a health savings account. This plan has two options:

a. 80 - this plan is for those who would like a health savings account with a lower deductible.
b. 100 - this plan is for those who want a health savings account and no coinsurance.

Added benefits, services and discounts with Blue Cross Policies:
SliverSneakers Fitnesss Program - Platnum Blue members receive free access to amenities such as treadmills, weights, heated pool and fitness classes that are included with a basic fitness membership. You can also participate in health education seminars and fun social events with others who share your interest in a healthy lifestyle. An alternative is the SilverSneakers program if you don't live near a fitness location. It is a self-directed physical activity program designed for older adults who want to increase their activity levels as part of a healthy lifestyle. Members can get fit by using the steps kit that includes a pedometer, exercise resistance bands, exercise DVD, and access to online features.

Fitness Program - Senior Gold members earn up to a $20 credit toward monthly health club fees when you exercise at a participating fitness center at least 12 times per month.

24-hour Nurse Advice Line - Friendly, knowledgeable nurses are available seven days a week.

Stop-Smoking Support - Take advantage of this proven, successful program. Trained phone counselors and support materials can help you quit.

Vision and hearing product discounts - Save on eyeware and hearing aids.

*Networks:
Aware Network – This is Blue Cross’s most extensive network. It includes 100% of hospitals and 98% of doctors in Minnesota

Accord Network – This network does not include the following hospitals/clinics: Mayo Clinic, Rochester; St. Mary’s, Rochester; Methodist, Rochester; and Hazelden.

Delta Dental - See any dentist you like, with the greatest savings at a network dentist. This coverage is for a broad spectrum of dental care. There is no waiting periods for routine exams, fillings and some more involved procedures. There are two plans to choose from, one with greater coverage and one with lower premiums.
